Submissions Release Form

I hereby permanently give Valiant Ray Publishing and their legal representatives and assigns the right and permission to publish my name, age, pictures, and story submitted for the specific purpose of my story being published in one of their books and any other media from the "Lessons in the Sky Collection".

I understand if my story is chosen, by signing this form, I give Valiant Ray Publishing my permission to publish my name, age, pictures and story for any use of marketing material, social media, publication on their website www.ValiantRayPublishing.com and permission for my story to be told in keynote speeches/public events in anyway necessary.

I understand if my story is chosen, I may be contacted with questions and for more details about my story submitted.

I understand Valiant Ray Publishing will not disclose my personal address, phone number or email.

I hereby release and hold harmless Valiant Ray Publishing, their owner, office, employees, agents, designees from any, and all responsibility or liability. I understand that I will receive no compensation, should any of my information or story of myself be used.

I understand Valiant Ray Publishing has full authority to my information, story and pictures submitted to be placed on their website, published in their book, and used for marketing materials/ social media purposes. Valiant Ray Publishing is not required nor obligated to use any of my information or story I submitted.
